(Fitness, Mind Body and Reiki Sessions/Classes)

 

By viewing and reading the foregoing, you are agreeing to accept all parts of this Hold Harmless and Assumption of the Risk Agreement/Waiver Release from Liability and Indemnity. Thus, if you do not agree to the Hold Harmless and Assumption of the Risk Agreement/Waiver Release from Liability and Indemnity, STOP now, and do not access or participate in online classes with Andrea G. Santana (Dre G. Santana, Dre Santana) .

 

For and in consideration of online participation with instructor Andrea G. Santana (Dre G. Santana, Dre Santana) (Cardio Dance, Cosmic Alignment + Yoga, Dance Class, Dance Discoteca, DANCE MAGIC DANCE!, Fluffy Fascia Land, Hatha Yoga, High Vibe Hip Hop Tribe, Hip Hop Yoga, Meditation + Heal, Meditation, Metaphysics & Yoga, Myofascial Release Class, Reiki, Restorative Yoga with Crystal Bowls, Roll & Restore, Roll & Restore One-on-One, Roll & Release, Tibetan Bowls Sound Meditation, Vinyasa Yoga, Yoga, Yin Yoga) (hereafter “Fitness Class/Session”), I hereby voluntarily release, discharge, waive and relinquish any and all actions or causes of action for any personal injury, property damage or wrongful death against Andrea G. Santana and/or any of her officers, agents, servants or employees, occurring to me as a result of my participation in the Fitness Class/Session or any activities incidental thereto wherever or however the same may occur and for whatever period said activities may continue.

 

To the maximum extent allowed by law, it is my intention by participating online (virtual class), to exempt and relieve Andrea G. Santana, her officers, agents, servants or employees from liability for personal injury, property damage or wrongful death caused by negligence. I am fully aware of the risks and hazards inherent in participation in the Fitness Class/Session and I certify that I am in good physical and mental health and able to participate in physical and meditative activities. I realize that no medical insurance is provided by Andrea G. Santana for any injuries that may occur to me during participation in the Fitness Class/Session. Nevertheless, I hereby elect voluntarily to participate in the Fitness Class/Session and assume all risk of loss, damage, or injury that may be sustained by me during my participation in the Fitness Class/Session or any activities incidental thereto.

 

I agree that in the event any claim for personal injury, property damage, or wrongful death shall be prosecuted against Andrea G. Santana, or any of her officers, agents, servants or employees as a result of my participation in the Fitness Class/Session, I shall defend, indemnify and save harmless Andrea G. Santana and/or any of her officers, agents or employees from any and all such claims or causes of action by whomever or wherever made or presented.

 

I understand that this Hold Harmless and Assumption of the Risk Agreement/Waiver Release from Liability and Indemnity shall apply not only to me but also to my heirs, executors, administrators, next of kin, assigns and successors.

 

I acknowledge that I have read this document and have been fully and completely advised of the potential risks and hazards incidental to participation in the Fitness Class/Session and I am fully aware of the legal consequences of reading this instrument. I understand its contents. I voluntarily agree to the terms and conditions stated above.
